Historical SAMHSA Treatment Facility Data
This project reconstructs address-level treatment facility listings from 26 SAMHSA directories: 1998, 2000, 2001, and 2003-2025. PDF directories through 2021 are converted to a common format aligned with SAMHSA’s 2022-2025 workbooks. Directory and survey years are retained.

These are historical records, not a current treatment locator. Historical phone numbers are included in research downloads and may be stale.

Limitations
Quality assurance is ongoing. Manual gold-sample review of the PDF years is incomplete. The 2022-2025 spreadsheet directories have passed checksum and structural checks but have not received the same record-level review. In the official 2025 workbook, 556 U.S. listings omit a street address while retaining city, state, and ZIP; these records remain included and flagged. County geography is preliminary; most assignments currently use a ZIP-to-county fallback. Missing characteristics mean that a question was not asked or cannot be harmonized, not that no facility offered the service. Pre-2021 N-SSATS and post-2020 N-SUMHSS data are not directly trend-comparable.
